LIST: These are the positive cases of coronavirus at North Texas nursing homes and state-supported living centers

So far in North Texas, there have been more than 1,000 positive cases of COVID-19 and 150 related deaths in long-term care facilities in the region.

Updated on June 7 with new information from the city of Arlington. 

There have been reports of positive COVID-19 cases at hundreds of nursing and assisted living facilities across Texas since the pandemic began, with numerous large outbreaks here in North Texas. 

More than 1,000 residents of such facilities have tested positive for the disease and more than 150 people connected to these facilities have died since tracking began in March.

Nearly 20,000 people in the Dallas-Fort Worth area have tested positive for the novel coronavirus in the same time-frame. The first local case was announced on March 9 in Collin County.

Below is a breakdown of known cases at North Texas nursing homes and state-supported living centers.

Dallas County

Number of cases: 414 residents, 118 staff members confirmed positive; 72 deaths

As of May 26, Dallas County health officials were reporting more than 500 cases associated with 28 long-term care facilities. In addition, there have been 73 deaths tied to the novel coronavirus at a number of those facilities, with the biggest number -- 20-- coming at Monticello West.

County officials have continued to point to nursing facilities as some of the most vulnerable. Last month, a spokesperson said the county does not have regulatory authority over Monticello, which has the deadliest outbreak, but has shared the CDC's recommendations.

The confirmed cases are broken down by facility as follows:

Monticello West

  • 43 residents, 3 staff members confirmed positive

Tarrant County

Number of cases: 540 cases, 38 deaths

Tarrant County health officials confirmed to WFAA on May 8 that there have been a total of 540 cases of COVID-19 connected to long-term care facilities since the pandemic began. 

Of the 540 cases, 414 were among residents of such facilities, while 126 staff members had been infected.

At least 38 people linked to such facilities in Tarrant County have died as of May 11.  

A total of 36 facilities have had cases, county officials report. WFAA has requested officials specify which facilities, but health officials have provided some updates from certain locations thus far. 

Advent Health Care Center in Burleson has had at least nine deaths and 58 total positive cases, according to Vinny Taneja, the county's public health director. 

Taneja said the health department has been working with the facility and HHSC for several weeks. 

“They have been very cooperative with us,” he said. “They’ve implemented everything that we’ve told them.” 

He also said that the facility has brought in the capacity to conduct testing.  

“There is really no reason why this facility has done any worse than the others,” Taneja said of Advent. “Call it, bad luck.” 

At the Fort Worth Transitional Care Center, at least 49 cases have been recorded.

Arlington city officials reported 44 new cases at Meadowbrook Memory Care Community on June 7, with 30 residents and 14 staff members having tested positive so far.

Denton County

Number of cases: 109 cases, 5 deaths

The Denton State Supported Living Center is a residential facility for people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. 

As of May 2, 54 residents and at least 50 staff members at DSSLC had contracted COVID-19.

Four people who were residents of the Denton Rehabilitation Center have died from the disease, according to county officials. Three of the four deaths were announced on April 11. They include a woman in her 80s, another woman in her 60s and a man in his 80s. On April 14, officials said a man over the age of 80 who was a resident at the facility had also died. 

Also in Denton County, a man in his 90s who was a resident of a Lewisville nursing home died from COVID-19, county officials said April 5. He had been hospitalized due to the illness.

Lamar County

Number of cases: 72 cases, 7 deaths

Officials announced on April 29 that the first person to die in the county from the disease had been associated with the current outbreak at the Paris Healthcare Nursing Home. 

By May 16, a seventh person connected to the Paris Healthcare Nursing Home had died.  There have been a total of 9 deaths in the county so far. The other two were not connected to nursing homes, officials said. 

The outbreak at the nursing home has made up the bulk of the county's 96 cases, with 54 reported cases so far linked to the outbreak there as of May 10. And another 15 cases have now been reported at the Stillhouse Rehabilitation and Healthcare Center, a different long-term care facility in Paris. Officials first reported cases there on May 5.

Three people connected with the Brentwood Terrace Healthcare & Rehabilitation Center have also now tested positive, officials said on May 7.

Both Brentwood and Stillhouse have additional test results pending, and of the county's 96 total cases, 72 have been linked to one of the three facilities.

Officials first addressed the outbreak at the Paris Healthcare Nursing Home on April 26 when they said, between staff and residents, 41 people associated with the home had tested positive for COVID-19.

All of those who have tested positive have been notified and immediately quarantined, according to officials, who are strongly recommending people wear masks in public. 

"This virus has seemed distant to most people in our community. It was happening elsewhere," Mayor Steve Clifford wrote on Facebook when the outbreak was first reported. "Now we know that many people who were infected with the virus and had no symptoms have been in numerous public places throughout our community. And most of the people in our community are still not wearing masks."

Clifford also announced that county health resources would be diverted to containing the spread from the "massive outbreak" at the nursing home, prompting further drive-thru testing scheduled for later this week to be postponed.

Rockwall County

Number of cases: 61 cases, 12 deaths

On April 26, officials announced the first death related to COVID-19 had occurred in Rockwall County. They would later reveal on May 5 that that person had been a resident at the Broadmoor Medical Lodge, a facility plagued by problems with the disease. 

Officials announced on May 21 there have now been 61 cases connected to the Broadmoor Medical Lodge. Forty of those cases are residents, 15 of whom have recovered so far. The other 21 are staff members; 17 have recovered. 

Twelve people connected to the facility have died, at least six of whom were residents.

The county's entire death toll is comprised of people connected to the nursing home.

Local fire departments are now working with the Broadmoor and other nursing homes to ensure those who are sick are getting tested.

The county has now had at least 168 total cases reported.

One important aspect to note, however, is that not all staff members live in the county, so not all of them will be included in the state's data on the county. 

The first round of testing at the facility had been completed by April 28, according to officials, which is when they announced the 20 residents had tested positive, along with an initial six staff members. That number of staff members would rise to 19 by May 5, as county officials said a second round of testing was underway at the facility.  

By May 9, the second round of testing had been completed and 12 new cases had been found among residents and one new case in a staff member, officials said, bringing the facility's case count to the current total of 52 people. 

As the facility continues to test its people, more have continued to test positive for the disease.

Ellis County

Number of cases: 34 cases, 8 deaths, 10 recoveries

As of May 12, at least 34 residents of Legend Oaks Healthcare and Rehabilitation have contracted COVID-19, Waxahachie officials said. 

At least eight residents had died from the disease as of May 12, including:

  • A 66-year-old man
  • A 70-year-old woman
  • A 71-year-old woman
  • A 79-year-old woman
  • An 80-year-old woman
  • An 85-year-old woman
  • A 92-year-old woman
  • A 93-year-old man

Ten residents have recovered from the disease as of May 12, including:

  • A 60-year-old man
  • A 63-year-old man
  • A 66-year-old woman
  • A 69-year-old woman
  • A 70-year-old woman
  • An 84-year-old woman
  • An 86-year-old woman
  • An 89-year-old woman    
  • Two 92-year-old women

There are 16 more residents whose cases were considered active as of May 12, including:

  • A 62-year-old man
  • A 65-year-old man
  • A 69-year-old woman
  • A 72-year-old woman
  • A 75-year-old man
  • A 77-year-old man
  • An 81-year-old woman
  • An 82-year-old woman
  • An 83-year-old woman
  • An 84-year-old woman
  • An 85-year-old man
  • An 85-year-old woman
  • An 87-year-old man
  • A 90-year-old woman
  • A 91-year-old woman
  • A 94-year-old woman

The facility is working with the state health department and is under monitoring by state authorities, Waxahachie officials said.

City and county personnel have offered their support, along with a supply of personal protective equipment.  

County health officials announced the facility's first case on April 6, an 86-year-old woman who had been moved from the facility to the care of Baylor Scott & White Medical Center in Waxahachie, where she was hospitalized in isolation.

Collin County

Number of cases: 17 deaths

At least 17 people who were connected to long-term care facilities in the county and had COVID-19 have now died, according to officials.  

They include:

  • An 82-year-old McKinney man who was a resident of North Park Health and Rehabilitation Center and died on May 6
  • A 92-year-old man who lived at the Life Care Center in Plano who died on May 8
  • A 90-year-old McKinney woman who was a resident of Grand Brook Memory Care and died on May 9
  • An 88-year-old man who was a resident of Grand Brook Memory Care whose death was announced on May 1
  • One other person linked to Grand Brook Memory Care
  • An 86-year-old McKinney woman who died at the Oxford Grand Assisted Living & Memory Care facility whose death was announced on May 7
  • An 85-year-old woman who lived at Oxford Grand Assisted Living & Memory Care died on April 23
  • A 95-year-old woman who was a resident at Oxford Grand Assisted Living & Memory Care died on April 24
  • 9 other deaths linked to the Oxford Grand Assisted Living & Memory Care facility
